WAIVER AND RELEASE OF LIABILITY BIXBY KNOLLS NEIGHBORHOOD ASSOCIATION 5K FUN RUN
In consideration of my participation in the Bixby Knolls Neighborhood Association (BKNA) 5K Fun Run, I hereby agree to the following:
1. Assumption of Risk: I understand that participating in this event involves inherent risks, including but not limited to physical injury or death. I voluntarily assume all such risks, known and unknown, and accept personal responsibility for any injury, loss, or damage that may result from my participation.
2. Release of Liability: I, for myself and my heirs, executors, administrators, and assigns, hereby waive, release, and discharge BKNA, volunteers, and agents from any and all claims, demands, or causes of action arising out of or related to my participation in the 5K Fun Run, even if such claims arise from the negligence of BKNA or its representatives.
3. Medical Fitness: I certify that I am physically fit and have not been advised otherwise by a qualified medical professional. I acknowledge that I should consult a physician before participating in any physical activity.
4. Photographic Release: I grant permission for BKNA to use photographs and videos taken during the event for promotional purposes without compensation to me.
5. Compliance with Rules: I agree to abide by all rules and regulations set forth by BKNA regarding the event.
I have read this waiver and fully understand its terms. I acknowledge that I am signing this waiver voluntarily and that my participation in the BKNA 5K Fun Run is contingent upon my acceptance of these terms
